Exiting Public Systems
Too often, people are discharged from a public system — like child welfare, prison, or a hospital — into homelessness. Involvement with public systems is a risk factor for homelessness, but one that can be mitigated through adequate discharge and transition planning that includes access to housing and supports. This is a key part of systems prevention, one of the five categories associated with homelessness prevention.

It is organized by system, recognizing that the needs of an adult exiting prison differ significantly from those of a young person aging out of foster care.
